Peak Rehabilitation Hospital is seeking a full-time Clinical Liaison for its Inpatient Rehabilitation unit. This role is crucial in driving appropriate admissions by building and maintaining referral relationships. The Clinical Liaison will assist with the intake and pre-admission screening process, perform on-site clinical judgments to determine patient appropriateness for admission, and communicate rehabilitation and facility options to patients and their families. They will also inform patients about the expectations and requirements for participation in the rehabilitation program and secure information regarding patients' resources and benefits. Additionally, the Clinical Liaison serves as the initial point of contact for external case management and discharge planner personnel with payors and hospital facilities. Peak Rehabilitation Hospital is a joint-venture partnership between Duke Health, WakeMed, and Lifepoint Rehabilitation, offering a state-of-the-art, 52-bed inpatient acute rehabilitation hospital dedicated to treating individuals who have experienced severe injuries or illnesses. The hospital provides customized, individualized rehabilitation for conditions such as stroke, brain injury, neurological conditions, trauma, spinal cord injury, amputation, and orthopedic injury, aiming to maximize health, function, and quality of life.
