About The Position

The Clinical Liaison serves as a key connection between Midland Care Connection, healthcare providers, patients, and their families. This role focuses on building relationships with hospitals, physicians, and other healthcare facilities to promote Midland’s services, coordinate referrals, and help patients access the care they need. Responsibilities include conducting patient assessments to determine eligibility, educating patients and providers about available programs, collaborating with the care team to coordinate individualized care plans, and maintaining communication throughout the care process. The Clinical Liaison also supports community outreach efforts, maintains accurate documentation, and helps ensure patients receive appropriate, high-quality care that meets their individual needs.

Requirements

  • Bachelor’s degree in Nursing, Social Work, Healthcare Administration, or a related healthcare field.
  • Current professional licensure or certification applicable to the role, as required.
  • Experience working in a healthcare setting with patients, families, and/or healthcare providers.
  • Strong communication, relationship-building, and customer service skills.
  • Ability to assess patient needs and coordinate services across multiple healthcare providers.
  • Strong organizational and time-management skills with the ability to manage multiple priorities.
  • Proficiency with electronic health records and Microsoft Office or similar computer systems.
  • Ability to maintain accurate, confidential documentation and comply with healthcare regulations.

Nice To Haves

  • Experience in care coordination, case management, hospice, home health, PACE, or other community-based healthcare services.
  • Experience working with hospitals, physicians, skilled nursing facilities, or other healthcare organizations.
  • Previous experience in a clinical liaison, healthcare outreach, referral development, or business development role.
  • Knowledge of community resources and healthcare services available to older adults and their families.
  • Experience with healthcare marketing, community outreach, or provider relationship development.
  • Knowledge of Medicare, Medicaid, and other healthcare payer/program requirements.
  • Strong understanding of interdisciplinary care and patient-centered care planning.
