About The Position

The Denver Hospice has an opening for a Clinical Liaison. This role acts as a liaison with assigned hospitals, facilities, and physicians, focusing on building effective relationships to promote communication, problem-solving, and positioning the affiliate as a premier high-quality hospice and palliative care provider. The position involves assessing and educating prospective patients and families about hospice services, determining hospice appropriateness, and assisting with admissions. It also includes assessing and counseling patients and families on their physical and psychosocial needs, gathering and recording information, and providing education on the hospice philosophy. The Clinical Liaison is responsible for completing admission paperwork, initiating clinical records via EMR, assessing and placing orders for medications, durable medical equipment, and oxygen as needed. The role also involves functioning as the primary RN for GIP patients in facilities, providing discharge planning assistance, and consulting with hospital, SNF, or ALF staff to facilitate discharges. Additionally, the Clinical Liaison interfaces with referring physicians, nurses, ancillary providers, and discharge planners to ensure a coordinated transition into hospice and communicates pertinent patient/family information to the interdisciplinary team. Rounding at assigned hospitals to educate and identify potential GIP candidates is also part of the role, along with other assigned duties.

Requirements

  • Graduate of an accredited school of nursing.
  • 1-2 years of experience in hospice and/or palliative care, medical/surgical, public health, oncology, acute care or home health.
  • Current Colorado RN licensure.
  • Valid Colorado State driver’s license and auto insurance.
  • Current CPR certification (for Pathways, The Denver Hospice and Pikes Peak affiliates). CPR must be provide and/or obtained within 14 days of hire.
  • Ability to lift minimum of 30 lbs.

Responsibilities

  • Acts as a liaison with assigned hospitals, facilities and physicians with an emphasis on building effective relationships to promote communication, problem-solving and positioning the affiliate as a premier high quality hospice and palliative care provider.
  • Provides assessment, supportive care and education to prospective patients and families regarding hospice services.
  • Determines hospice appropriateness of the patient; assists in admitting patients to the hospice program.
  • Assesses and counsels the patients and families during the initial interviews to properly determine the level of care as well as the physical and psychosocial needs of the prospective patient.
  • Accurately gathers information and data, reviews charts, and records information.
  • Provides education on hospice philosophy.
  • Efficiently and accurately completes all admission paperwork and initiates the clinical record through the EMR process demonstrating knowledge of the admission protocol.
  • Assesses and places orders for immediate medication needs.
  • Oversees the placement orders for durable medical equipment, supplies and oxygen as needed.
  • Functions as primary RN for GIP patients in facility.
  • Provides discharge planning assistance to appropriate staff.
  • Consults with staff of hospital, skilled nursing facility (SNF) or Assisted Living Facility (ALF) to determine discharge orders and to facilitate discharge to home, SNF or other residence.
  • Interfaces with referring physicians, nurses, ancillary providers and discharge planners to facilitate a well-coordinated transition into hospice for patients and families.
  • Communicates pertinent patient/family information to the interdisciplinary team and follows through to facilitate a smooth transition
  • Rounding at the assigned hospital to help educate and identify patients that qualify for General Inpatient services when appropriate.
  • Other duties and responsibilities as assigned.
