About The Position

Select Rehab of Palm Beach - Acute Rehab Unit is seeking a Clinical Liaison (RN / PT/ OT/ SLP) to join their team. This role involves advocating for referred patients in the pre-admission process by collaborating with clinical teams at referring hospitals to assess patient needs. The Clinical Liaison will serve as a resource for healthcare professionals by fostering relationships within medical facilities. Utilizing clinical expertise and problem-solving skills, they will also coordinate care plans with physicians and families. The position is located at 3060 Melaleuca Lane, Lake Worth, Florida.

Requirements

• RN, PT, OT, or SLP license
• Ability to complete clinical assessments of medically complex patients.

• Superior communication and relationship-building skills.
• Prior healthcare marketing or business development experience is preferred, but extensive training is provided on this piece.

Responsibilities

• Work with physicians, discharge planners, and patients' families to plan the best continuum of care after an ICU or post-surgical stay by utilizing strong clinical background, payer knowledge, and problem-solving skills.
• Perform clinical assessments of referred patients, then counsel with patients' families to assist them in making the best care decisions for their loved one.
• Partner with hospital case managers and physicians to create a smooth pre-admission and transfer process through removing payer-related and operational barriers for patients getting into our highly effective levels of care.
