About The Position

Clinical Liaison (RN / RRT) for Select Specialty Hospital Critical Illness Recovery Hospital (LTACH). This position involves traveling daily to referring hospitals near Lexington, KY, and once per month to the hospital in Danville, KY. The role focuses on advocating for referred patients, collaborating with clinical teams, assessing patient needs, serving as a resource for healthcare professionals, fostering relationships within medical facilities, coordinating care plans, facilitating patient transfers, and addressing barriers to specialized post-ICU care.

Requirements

• Registered Nurse (RN) or Respiratory Therapist (RT) License.
• Ability to complete clinical assessments of medically complex patients.

Nice To Haves

• Will consider other designations like LPN, PT, OT, etc., upon further review.
• Demonstrated success in patient advocacy.
• Superior communication and relationship-building skills.
• Prior healthcare marketing or business development experience is preferred, but extensive training is provided.

Responsibilities

• Advocate for referred patients in the pre-admission process by collaborating with clinical teams to assess patient needs.
• Serve as a resource for healthcare professionals by fostering relationships within medical facilities.
• Coordinate care plans with physicians and families using clinical expertise and problem-solving skills.
• Facilitate patient transfers and address barriers to accessing specialized post-ICU care.
• Work with physicians, discharge planners, and patients' families to plan the best continuum of care after an ICU or post-surgical stay.
• Perform clinical assessments of referred patients and counsel with patients' families to assist them in making the best care decisions.
• Partner with hospital case managers and physicians to create a smooth pre-admission and transfer process by removing payer-related and operational barriers.
