You will bridge the gap between hospital leadership, medical staff, and patients to ensure a seamless transition to myLaurel’s high-acuity home care model. Requirements

  • Current and unrestricted RN License in Washington State (or Compact State).
  • Minimum of 5 years of clinical experience in an Inpatient or Emergency Department setting.
  • A proven track record of launching or scaling new healthcare services or programs.
  • EHR proficiency and the ability to navigate complex care systems and hospital workflows.
  • Exceptional interpersonal skills with the ability to influence hospital leadership and medical staff.
  • Bachelor’s degree in Nursing (BSN) or a related healthcare field is required.

Responsibilities

  • Attend hospital "round huddles" to identify high-risk patients who would benefit from acute care at home.
  • Collaborate with Client Success to support program performance aligned with the hospital's clinical quality, readmission reduction, and throughput objectives
  • Educate patients and caregivers on the myLaurel program, ensuring a smooth transition and clear expectations for home visits.
  • Conduct census and chart reviews to identify patients who meet clinical criteria for the acute care at home program, supporting appropriate care transitions in coordination with hospital care teams
  • Serve as the clinical expert, educating hospital staff and providers on program benefits and outcomes.
