Requirements

Current and unrestricted RN License in Washington State (or Compact State).
Minimum of 5 years of clinical experience in an Inpatient or Emergency Department setting.
A proven track record of launching or scaling new healthcare services or programs.
EHR proficiency and the ability to navigate complex care systems and hospital workflows.
Exceptional interpersonal skills with the ability to influence hospital leadership and medical staff.
Bachelorâs degree in Nursing (BSN) or a related healthcare field is required.

Responsibilities

Attend hospital "round huddles" to identify high-risk patients who would benefit from acute care at home.
Collaborate with Client Success to support program performance aligned with the hospital's clinical quality, readmission reduction, and throughput objectives
Educate patients and caregivers on the myLaurel program, ensuring a smooth transition and clear expectations for home visits.
Conduct census and chart reviews to identify patients who meet clinical criteria for the acute care at home program, supporting appropriate care transitions in coordination with hospital care teams
Serve as the clinical expert, educating hospital staff and providers on program benefits and outcomes.
