About The Position

Salus Hospice is seeking a dynamic and clinically knowledgeable Clinical Liaison to serve as the primary connection between hospitals, physicians, and our hospice care services. This role is critical in ensuring seamless patient transitions from acute care settings to home-based services, while maintaining strong relationships with referral sources and delivering exceptional patient-centered care. Salus Hospice is a Joint Commission-accredited provider offering comprehensive home health, palliative, and hospice services. Requirements

  • Active LVN or RN license in the state of California
  • Minimum 1–2 years of experience in hospice, palliative care, home health, or healthcare marketing/liaison role
  • Strong understanding of Medicare and insurance guidelines
  • Excellent communication and relationship-building skills
  • Ability to work independently in a fast-paced, field-based environment

Nice To Haves

  • Prior experience as a Clinical Liaison or in business development within healthcare
  • Knowledge of San Diego healthcare systems and referral networks
  • Experience in interdisciplinary care coordination

Responsibilities

  • Coordinate and facilitate smooth transitions from hospital or facility to home care
  • Reduce re-hospitalizations through proactive discharge planning
  • Ensure continuity and accuracy of patient care plans
  • Assess patient eligibility for home health, palliative, or hospice services
  • Collaborate with physicians and care teams to determine appropriate levels of care
  • Support development and execution of individualized care plans
  • Develop and maintain strong referral relationships with hospitals, physicians, and healthcare providers throughout San Diego
  • Serve as a trusted clinical resource for partners and families
  • Represent Salus in the community to promote service awareness
  • Maintain knowledge of Medicare, insurance guidelines, and hospice eligibility criteria
  • Ensure all admissions and referrals meet regulatory standards
