About The Position

The Registered Nurse (RN) Clinical Liaison acts as the key connection between Arden Hospice and acute care hospital partners. This patient-facing role is clinically focused and responsible for ensuring safe, efficient, and well-coordinated transitions of care from the hospital to the home setting. Reporting to the Regional Sales Director, and working in close collaboration with Clinical Operations, the Clinical Liaison assesses patients for hospice eligibility, provides education to patients and families regarding post-acute care options, partners with hospital discharge planning teams, and facilitates a smooth transition to field-based clinical staff. This role plays a vital part in supporting positive patient outcomes while contributing to the organization's growth and referral development.
