Requirements
- Associate degree or diploma in Nursing
- Two (2) years intravenous therapy experience in hospital or homecare
- Excellent patient assessment skills
- RN or LPN license in good standing in the state(s) where providing care / services
- Valid driver’s license
- Excellent driving record

- Knowledge of JCAHO accreditation process and overall JCAHO standards
- Working / practical knowledge of home infusion therapy administration / equipment use
Responsibilities
- Gathers and communicates demographic, clinical, and reimbursement information on referred patients to the intake department.
- Develops relationships and identifies opportunities to secure all potential home infusion referrals.
- Communicates insurance verification results to discharge planners and/or patients, assisting the reimbursement department.
- Assesses patient and caregiver needs and abilities, participating in developing and implementing patient-specific training and care plans.
- Facilitates discharge or referral processes by relaying necessary orders and documentation to appropriate NELC departments.
- Serves as a clinical liaison between NELC and hospital departments (medical staff, care coordination, social services, pharmacy, nursing).
- Monitors the status of hospitalized NELC patients and communicates updates to relevant NELC staff.
- Maintains a patient caseload averaging between 15 and 20 patients.
- Documents all interactions with patients and healthcare team members comprehensively in the patient record.
- Effectively communicates patient requirements and status changes to NELC infusion and reimbursement teams, as well as Home Health Agency (HHA) staff.
- Participates in daily patient care meetings and case conferences as required.
- Participates in hospital care management team rounds to identify and manage potential infusion patients, providing advice to the hospital care team.
- Completes therapy-specific reimbursement checklists accurately (e.g., Medicare TPN, ENT, Cardiac).
- Assists in developing and implementing new programs and educational seminars to enhance NELC's community healthcare services.
- Collaborates with Partnership Development staff to identify and capture new business.
