About The Position

The Clinical Leader of our local Bayshore Neighborhood Health Team (NHT) will have the opportunity to lead a team of healthcare providers supporting clients with in-home care services working hand-in-hand with a network of doctors and local health and wellness providers. This is a hands-on community health position where you will provide a personalized approach to home care for people who wish to age in place their own way. Our approach is uniquely nurse-led, allowing our expert and dedicated nurses to be actively involved in clients’ health and wellness journeys, and designing care plans that are tailored to fit each client’s personal lifestyle.

Requirements

  • A current and valid license in Ontario as a Registered Nurse (RN); an Occupational Therapist (OT); a Physiotherapist (PT) or Registered Social Worker (RSW).
  • A minimum of five (5) years of healthcare experience, preferably in a community setting.
  • A minimum of two (2) years of leadership experience and managing others.
  • Current and valid CPR certification.
  • A valid driver’s license and reliable vehicle is required to conduct home visits with clients within the community (roughly 60%25 in the field, vehicle allowance is provided).

Nice To Haves

  • Experience working with community healthcare, the geriatric population and their families is strongly preferred.

Responsibilities

  • Develop comprehensive plans for clients and families using a holistic approach to help them achieve their aging in place goals, and continue to resolve client’s challenges by offering services, products, advice, and community resources as their needs change.
  • Direct and monitor the delivery of all in-home services and daily functions of field staff providing access to clinical advice, support, and resources.
  • Hire and train all healthcare staff and supervise the day-to-day performance of the team; ensure exceptional interactions with prospective and existing clients; and provide ongoing coaching and performance management.
  • Manage expectations with family members and clients ensuring a client and family-centered approach to care planning.
  • Adapt care to changing client needs including occasionally working with the interdisciplinary team for management of acute illness, falls, and other immediate challenges.
  • Ensure shift coverage for callouts and sick calls, including providing direct hands-on care for clients.
