About The Position

As part of a mission focused, innovative, clinical program, the Clinical Intake Coordinator will work within an interdisciplinary team to provide services to youth involved in the juvenile justice system. This will include working with treatment staff and court personnel to coordinate care for clients in the Lighthouse Individualized Docket Services (LIDS) program. The staff member will work closely with possible clients, Lighthouse staff, court personnel, and other treatment providers to meet client needs and access to services.

Requirements

  • Professional licensure in State of Ohio as a Counselor, or Social Worker
  • One year of professional social service experience preferred
  • Valid driver’s license and good driving record
  • Knowledge of or willingness to learn and apply trauma-informed care principles

Nice To Haves

  • Masters in Counseling or Social Work Preferred

Responsibilities

  • Completes intake and initial diagnostic assessments for youth, determining needs and appropriate level of care.
  • Documents services with basic understanding of compliance and billing for regulatory bodies.
  • Initiates services within the expected timeframe as set by agency standards.
  • Completes progress notes and service documentation per agency standards.
  • Meets with probation officers for possible client referrals gaining access to the Lighthouse Individualized Docket Services (LIDS) program
  • Assist the family to negotiate the referral process for more efficient and effective engagement on the docket
  • Identify clients that best meet referral criteria and work with families and court personnel so that paperwork is properly completed
  • Work collaboratively with agencies, courtroom personnel, and family members involved with Juvenile Court
  • Assist the court to screen and process clients for LIDS program, including conducting initial screens with clients and family and integrate into the court functions as much as possible
  • Report findings to LIDS team and court personnel
  • Attend various court meetings including plea hearing, disposition hearings, and others as required
  • Complete follow up with all families who have completed the program
  • Track client referrals through the process until acceptance onto the docket and track appropriate metrics for client referrals as well as data for quality improvement and outcomes
  • Facilitate communication between service providers, Judges, LIDS team, and court personnel
  • Gather relevant records for client’s admission to program
  • Complete and maintain documentation of services
