About The Position

We are seeking a Clinical Improvement Nurse to support population health initiatives and improve clinical outcomes for members with chronic conditions. This role focuses on implementing evidence‑based interventions, supporting quality improvement programs, and promoting wellness, disease management, and patient education across the organization. The Clinical Improvement Nurse works closely with providers, staff, and members to improve health outcomes, support NCQA and Medicare Five Star performance standards, and enhance chronic disease management strategies. This role also plays a key part in patient education, care coordination, and continuous improvement initiatives driven by data and performance metrics.

Requirements

  • Associate's degree in Nursing from an accredited college
  • Active Registered Nurse (RN) license in the State of Florida
  • Current CPR certification
  • Minimum of five years of related clinical experience
  • Experience working with patients managing chronic conditions
  • Ability to analyze clinical data and implement improvement interventions
  • Strong patient education and communication skills
  • Ability to document care and maintain accurate records within clinical systems
  • Ability to collaborate effectively with providers and interdisciplinary teams
  • Ability to pass a background check
  • Ability to pass a drug screening test

Nice To Haves

  • Experience in population health, disease management, or quality improvement programs
  • Familiarity with NCQA standards, Medicare Five Star programs, or similar performance frameworks
  • Experience educating providers or staff on evidence-based care practices
  • Public speaking or group education experience
  • Experience working with electronic medical records (EHR) and clinical data systems
  • Experience participating in community outreach or wellness initiatives
