About The Position

Mercy Home Health, a member of Trinity Health At Home, is seeking a Clinical Home Care Coordinator for the Bucks County area at St. Mary's Hospital. This role is responsible for the efficient and effective facilitation of referrals to the post-acute homecare/hospice agency, providing education to acute care staff, and potentially engaging in business development activities. The ideal candidate will be an RN, LPN, or therapist (Licensed Social Worker, OT, or PT) preferred.

Requirements

Bachelor’s degree or equivalent level of experience required.
Graduate of an approved clinical education program and current clinical license in the state of Pennsylvania preferred.
Must have a minimum of two (2) years' experience in an acute care setting, Emergency Room setting, customer service, sales/marketing role or relevant experience.
Must be available to work 1 Saturday and Sunday a month.
Must have current Driver’s license and reliable transportation to and from work site.
Ability to consistently demonstrate a commitment to the mission and Organizational Code of Ethics and adhere to the Compliance Program.
Well-developed oral and written communication skills.

Responsibilities

Maintains daily contact with case managers/discharge planners and units to offer assistance and assure that patients referred for services are followed up on in a timely and efficient manner.
Coordinates care for clients going home from the hospital who have chosen the agency’s services.
Reviews and gathers information from the hospital medical record, copying portions for transmission as needed.
Documents initial assessment of homecare needs and potential plan for services in compliance with agency policy.
Arranges service in accordance with the established admission criteria, client care needs and client preference.
Obtains physician orders for initiation of homecare/hospice services in accordance with agency policy/procedure and third-party regulations.
