About The Position

Community Health Plan of Imperial Valley (CHPIV) is seeking a part-time clinician to provide home-based health assessment services for its Medicare Advantage Prescription Drug (MAPD) members. This role supports risk adjustment and quality of care. The position is a flexible, part-time contract role where the contractor has full discretion over the method, schedule, and manner of performing services, adhering to professional standards and regulatory obligations. Contractors are not eligible for employee benefits, unemployment insurance, or workers’ compensation.

Requirements

  • Licensed in California and in good standing as a Nurse Practitioner, Physician Assistant or Doctor of Medicine.
  • Maintain professional liability insurance (malpractice) and other insurance as required under applicable state law and professional regulatory standards.
  • Provide certificates of coverage to the Client upon request.

Responsibilities

  • Conduct yearly in-home health visits for CHPIV Medicare members who have not had an annual wellness exam with their primary doctor. These visits last about 45-60 minutes.
  • Perform initial assessments including review of current symptoms, medical history, and medications.
  • Conduct physical examinations, measuring vital signs and performing checks of the patient’s lungs and heart.
  • Perform preventive screenings for conditions like cardiovascular risk, diabetes, cognitive impairment, depression, or fall risk, and include eye and foot examinations as appropriate.
  • Review medications to identify potential interactions, confirm dosages, and assist with refills.
  • Conduct on-site diagnostics such as blood draws, ECGs, blood sugar testing, or other laboratory tests when available and appropriate.
  • Assess the living environment for safety hazards that could affect health.
  • Provide health counseling on diet, nutrition, exercise, and chronic disease management.
  • Coordinate care by communicating with the patient’s primary care physician (PCP) and CHPIV care manager as needed.
  • Document assessments and diagnoses accurately in CHPIV’s electronic health record (EHR) system.
  • Code diagnoses consistent with CMS guidelines and risk adjustment protocols based on Hierarchical Condition Categories (HCCs).
  • Maintain compliance with all federal, state, and local laws, including HIPAA, and applicable regulatory requirements.
