About The Position

Under the direction of the Care Coordination Supervisor or Field Instructor, the Clinical Float I supports Care Coordination and advocacy efforts for individuals enrolled in HCBS programs. This position will assist with care coordination and waiver service coordination duties without being assigned a dedicated caseload. Clinical Float 1 will be overseen by the appropriately licensed staff member.

Requirements

  • Licensed Practical Nurse in the State of Ohio or approved Social Work Trainee registration.
  • One year of experience in home care, geriatrics, behavioral health, or long-term care.
  • Valid Ohio driver’s license and auto liability insurance.

Responsibilities

  • Conduct routine in-person visits within required timeframes to ensure health, safety, and service satisfaction.
  • Develop and update person-centered care plans within required time frame.
  • Educate individuals and caregivers on health promotion, illness prevention, community resources and navigating the healthcare system.
  • Provide telephonic coverage as needed, including collaborating with Care Coordinator, Waiver Service Coordinators and service providers.
  • Assist with care coordination and advocacy for enrollees.
  • Conduct in-person assessments and significant change event assessments within required timeframes.
  • Schedule and participate in Interdisciplinary Care Team (ICT) meetings and complete assessments as required.
  • Educate members on waiver services and available providers, including self-direction and transportation needs.
  • Develop and document the Member's disaster preparedness/backup plan and contact information in the Waiver Service Plan.
  • Complete all actions and follow up related to critical and reportable incidents as defined in regulatory requirements.
  • Develop a health and safety action plan (HSAP) and monitor as applicable.
  • Consult Clinical Float 2 as needed.
  • Monitor service quality and program eligibility.
  • Perform data entry and handle incoming communications for Care Coordinators/WSCs.
  • Maintain confidentiality in accordance with HIPAA and Agency policies.
  • Collaborate with interdisciplinary teams including WSCs, Care Coordinators, physicians, and service providers.
