About The Position

This position is responsible for supervising, assisting and counseling all Cardiac Rehabilitation, and HealthStyles members for the purpose of developing individualized exercise prescriptions based on patient and member goals. This position is also responsible for orientation of exercise equipment, proper implementation of exercise prescription, and counseling of clients on risk factor modification and the physiological assessment of members.

Requirements

  • Bachelor’s Degree from a four-year college or university in exercise related field required.
  • One to two years related experience and/or training preferred.

Nice To Haves

  • Cardio-Pulmonary Resuscitation Certification and AED Training preferred at time of hire, required within 6 months of hire date.

Responsibilities

  • Consult with inpatient referrals post-op stent placement from catheterization lab/inpatient floor and outside referrals regarding outpatient cardiac rehabilitation program.
  • Facilitate implementation to Phase II OP cardiac rehab through referring physician.
  • Provide program benefits, eligibility, care team involvement and operation details to patient.
  • Educate patient on cardiovascular risk factors and provide literature.
  • Coordinate patient follow-up plan intake evaluation.
  • Notify referring physician to obtain an order and/or patient refusal based on barriers to enrollment.
  • Initiate intake/enrollment processes, including scheduling and follow-up.
  • Obtain pertinent patient medical records, including office visits, testing, medications, and contraindications.
  • Obtain insurance coverage for reimbursement based on qualifying diagnosis and procedure codes prior to patient enrollment.
  • Conduct intake and assess performance metrics related to outcomes measures.
  • Perform and document physical, psychosocial, and behavioral assessments.
  • Identify appropriate interventions and complete individualized treatment plan.
  • Establish individual realistic patient goals based on SMART goal setting template.
  • Maintain open communicate to team regarding patient or scheduling concerns.
  • Complete the daily operations of the Cardiac Rehabilitation Department.
  • Assess rest, exercise and recovery heart rate and rhythm, ratings of perceived exertion, blood pressures, blood glucoses, and O2 sats.
  • Develop exercise prescriptions according to age, disease, medical history, comorbidities, and individual needs for patients.
  • Provide orientation to exercise program, educating patient on facility guidelines and untoward signs and symptoms to recognize and report.
  • Safe set-up and use of exercise equipment and program processes.
  • Follow exercise progression protocols based on AACVPR risk stratification, including THRR.
  • Perform EKG and telemetry monitoring for Cardiac Rehabilitation patients.
  • Proficient in telemetry system utilization for patient enrollment, monitoring, and reports.
  • ECG interpretation skills and advanced knowledge of exercise physiology in clinical populations.
  • Properly detect arrhythmia and ischemia and/or changes.
  • Determine patient’s heart rate response to various exercises throughout the exercise session.
  • Assist with charging appropriate CPT codes for daily attendance.
  • Instruct patient in all components of risk factor modification related to individualized treatment plan.
  • Design, implement, and oversee patient individualized exercise plans.
  • Provide routine patient education on topics such as disease management, safe exercise principles, risk factor modification, medication compliance, stress management, and behavior modification.
  • Collaborate with interdisciplinary teams to align care strategies.
  • Demonstrate awareness of patient’s readiness to change and ability to learn.
  • Appropriately respond to emergencies.
  • Answering phone, taking messages.
  • Taking blood pressure of members.
  • Provide a safe and clean work environment.
  • Provide clinical insight/exercise education to medical fitness members.
  • Support cross-functional facility operations and floor coverage of the fitness center.
  • Perform other related duties as assigned or required.
  • Follow standard precaution guidelines.
