About The Position

The Clinical Exercise Physiologist will provide cardiac rehabilitation services through patient assessment, education, treatment planning, and supervised exercise sessions. This role involves coordinating referrals, intake scheduling, and new patient orientations while developing individualized care plans. The position also serves as a case manager for assigned patients, monitoring progress, modifying treatment plans as needed, tracking outcomes, and coordinating care with physicians and other healthcare professionals. Responsibilities include monitoring patient safety during rehabilitation sessions, facilitating individual and group education programs focused on cardiovascular health and lifestyle modification, and supporting related wellness initiatives. The role also requires participation in departmental meetings, patient care conferences, and quality improvement activities.

Requirements

  • Bachelor's Degree in Exercise Physiology or related field required.
  • Current BLS certification.
  • Interpretive/analytical skills.
  • Effective interpersonal communication skills.
  • Organizational skills.
  • Microsoft Office software knowledge.

Nice To Haves

  • Master's Degree in Exercise Physiology preferred.
  • Certification through ACSM (Certified Clinical Exercise Physiologist) or AACVPR (Certified Cardiac Rehabilitation Professional or Pulmonary Rehabilitation Certificate) required within 3 years (if no Master's Degree).
  • Advanced Cardiac Life Support (ACLS) preferred.

Responsibilities

  • Provide cardiac rehabilitation services through patient assessment, education, treatment planning, and supervised exercise sessions.
  • Coordinate referrals, intake scheduling, and new patient orientations.
  • Develop individualized care plans.
  • Serve as a case manager for assigned patients, monitoring progress, modifying treatment plans as needed, tracking outcomes, and coordinating care with physicians and other healthcare professionals.
  • Monitor patient safety during rehabilitation sessions, including vital signs, EKGs, symptoms, and blood glucose testing when appropriate.
  • Facilitate individual and group education programs focused on cardiovascular health and lifestyle modification.
  • Support related wellness initiatives, including weight management services, fitness assessments, group exercise programs, occupational health testing, community outreach, and student training as needed.
  • Participate in departmental meetings, patient care conferences, and quality improvement activities.
  • Perform patient assessment/monitoring: biometric data, EKG, medical record/patient history review, medication reconciliation, signs and symptoms, exercise tolerance, mental health status, quality of life, dietary habits.
  • Conduct exercise testing: submaximal and exercise tolerance testing, muscular strength and endurance, flexibility.
  • Develop exercise prescription and treatment plans.
  • Implement treatment plans/exercise, identifying needed exercise modifications and making changes per the changing needs of the patient.
  • Lead exercise training, communicating proper execution of the exercise and use of equipment, and communicating safety strategies.
  • Identify educational needs, communicate health information, facilitate attendance to group education classes, patient goal setting and engagement, and use strategies like motivational interviewing and active listening to facilitate successful behavior change.
  • Document and report patient's clinical status and response to exercise training in the medical record.
  • Complete regular progress reporting to referring providers and medical director.
  • Maintain a safe environment by participating in infection control procedures and reporting any equipment malfunctions or environmental concerns in a timely and appropriate manner.
