About The Position

Acentra Health is seeking a Clinical Evaluator (PASRR) in Omaha, Nebraska. This hybrid role involves conducting in-person preadmission evaluations and assessments in Omaha and surrounding areas. The primary responsibility is to perform face-to-face PASRR Level II assessments to determine appropriate placements and next levels of care for individuals with Serious Mental Illness (SMI), Intellectual Disabilities (ID), or Related Conditions (RC) in nursing homes, hospitals, and community-based settings. This PRN hybrid role offers flexibility, combining remote work from a home office with local travel, with travel expenses reimbursed. Acentra Health will provide necessary technology equipment for the home office, including a laptop.

Requirements

  • Must possess an active, unrestricted Nebraska license in one of the following: Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW), Licensed Marriage and Family Therapist (LMFT), Licensed Independent Clinical Social Worker (LICSW), Licensed Clinical Psychologist (LCP), Registered Nurse (RN), or another qualified Licensed Mental Health Professional (LMHP).
  • Graduation from an accredited college with a relevant degree program and minimum of 2 years of experience assessing individuals with mental health or psychiatric disorders, including complex differential diagnoses related to substance use, medical conditions, cognitive impairments, and dementia.
  • Skilled in assessing and analyzing psychological, emotional, behavioral, and physical functioning, with basic knowledge of standards and best practices for determining Level of Care within mental health, geriatrics, and conditions commonly requiring long-term care placement.
  • Strong verbal, written, and electronic communication skills with the ability to engage effectively with individuals and groups.
  • Detail-oriented with strong organizational skills and the ability to prioritize and complete tasks efficiently within established timelines.
  • Familiarity with public mental health and medical programs.
  • Proficient in computer-based software and tools, including Microsoft Office Suite (Word, Excel, Outlook, Teams).

Nice To Haves

  • Basic knowledge of Preadmission Screening and Resident Review (PASRR) requirements, including applicable federal regulations and state-specific rules, is a plus.
  • Previous experience working with the geriatric population is preferred.

Responsibilities

  • Conducting PASRR (Preadmission Screening and Resident Review) assessments and evaluations, and developing informed level-of-care recommendations across diverse settings—including nursing homes, hospitals, and community environments.
  • Determining the appropriate next level of care for individuals who may require long-term care placement, while effectively assessing each individual’s unique needs.
  • Adhering to the state's best standards of mental health practices, intellectual and developmental disabilities, geriatrics, and other conditions requiring nursing care services and supports.
  • Ensuring compliance with contractual and regulatory standards while developing level of care recommendations.
  • Utilizing independent judgment, deep clinical expertise, exceptional communication skills, and problem-solving abilities to navigate complex situations, ensuring the best outcomes for members, providers, and clients.
  • Fostering effective collaboration with a diverse range of healthcare professionals, patients, families, and support networks while monitoring personal metrics to ensure compliance with turnaround time requirements.
  • Staying informed on current literature, regulations, and research pertaining to long-term care, mental illness, medical conditions, and geriatrics.
  • Leveraging technology by utilizing the state's electronic system and our proprietary platforms to efficiently manage and track all assessment activities.
  • Executing tasks with strong attention to detail, ensuring adherence to established workflows to maintain high standards of quality and operational efficiency.
