About The Position

As a Support at Home Care Partner Enrolled Nurse, you'll play a vital role in supporting older people to remain safe, independent and well in their own homes. Working under the direction of a Registered Nurse Clinical Care Partner, you will deliver person-centred nursing care aligned with clinical standards, regulatory requirements and best practice in community aged care. This role blends clinical nursing with meaningful community connection - supporting clients, their carers, and a multidisciplinary team to achieve the best possible health and well being outcomes.

Requirements

Enrolled Nurse registered with AHPRA and hold a current practising certificate.
Minimum of three years' experience in nursing, ideally within aged care or community settings.
Confident in delivering person-centred care.
Ability to prioritise workloads.
Ability to recognise and escalate changes in a client's condition.
Strong communication skills.
Ability to work effectively within a team environment.
Treat older people and their carers with empathy, dignity and respect.
Calm, organised and responsible approach to work.
Genuine commitment to supporting independence and positive health outcomes at home.
Satisfactory Criminal History Check.
Appropriate immunisation requirements.
DHS Working With Children Check (WWCC).
DHS National Disability Insurance Scheme (NDIS) Worker Check.
National Police Certificate (NPC) for employment involving unsupervised contact with vulnerable groups.
