About The Position

The Registered Nurse (RN) – Clinical Education & Care Transitions Coordinator is responsible for coordinating the clinical aspects of resident admissions and discharges while overseeing nursing staff education and competency development. This role ensures safe and efficient transitions of care, regulatory compliance, and accurate clinical documentation across the continuum of services. The RN collaborates with the interdisciplinary team to support person-centered care planning, coordinate admission and discharge processes, and ensure continuity of care for residents transitioning between healthcare settings. In addition, the RN plans, implements, and evaluates staff development programs including orientation, required in-service education, and competency validation for nursing staff. This position supports regulatory compliance with federal and state requirements, participates in quality assurance and performance improvement (QAPI) activities, and promotes clinical best practices through education, coaching, and collaboration with nursing leadership. The RN serves as a clinical resource to staff and contributes to a culture of safety, accountability, and excellence in resident care.

Requirements

  • Current, unencumbered, active license to practice as a RN in this State.
  • BLS CPR Certification (includes in-person demonstration and skills assessment).
  • Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities and infection control.
  • Must have excellent organizational, analytical, interpersonal, and project planning skills as well as strong verbal and written communication skills
  • Minimum One to three (1-3) years’ experience in geriatrics or post-acute care preferred.
  • One – three (1-3) years of teaching experience preferred.
  • Must be listed with a valid registration with a Nursing License in the state of North Carolina or reciprocal compact state.
  • Proof of Influenza vaccine and/or approved medical/religious exemption is required for all employees.

Responsibilities

  • Coordinates the clinical aspects of the admission process, including collecting and documenting resident data upon admission; completing medication reconciliation per facility policy; transcribing, verifying, and processing provider orders as directed; ensuring documentation is completed timely and accurately in the EMR; participating in admission care conferences as schedule; and communicating with residents and families regarding admission procedures.
  • Coordinates the discharge process in collaboration with the interdisciplinary team, including completing the discharge medication reconciliation per policy; preparing discharge documentation and instructions; arranging follow-up appointments and transportation; and communicating discharge plans with resident, family, and receiving providers.
  • Coordinates the laboratory orders from initiation through the receipt of results, including ensuring specimens are obtained timely and as ordered; tracking and logging laboratory orders and results; immediately reporting abnormal or critical results to the provider; and ensuring provider follow-up documentation is complete.
  • Performs routine audits as assigned and reports findings to DON or designee, including medication cart audits, crash cart audits per facility schedule; EMR audits as assigned; and assisting in reviewing care plan documentation for completion and timeliness.
  • Schedules and coordinates transportation for medical appointments, including ensuring the appropriate documentation and communication of appointments and ensuring follow-up documentation is obtained and reviewed after appointments.
  • Coordinates and schedules required in-service education annually and as needed for employees working in licensed areas of the community, securing qualified trainers when necessary.
  • Develops, implements, communicates, and evaluates the annual staff education plan and training calendar based on regulatory requirements, quality assurance data, and identified learning needs.
  • Collaborates with Department Directors to ensure staff participation in required training and completion of mandatory education.
  • Coordinates orientation and clinical competency validation for new nursing staff to ensure clinical requirements are met.
  • Maintains accurate records of education, competencies, and in-service attendance to ensure compliance with facility policies and regulatory requirements.
  • Develops and delivers education programs, teaching plans, and competency validation processes for all job roles and shifts in licensed areas of the community.
  • Works in collaboration with the Director of Nursing to promote quality resident care and ensure adherence to facility policies, procedures, and clinical standards.
  • Maintains clinical competency and provides nursing care as needed, utilizing the nursing process and collaborating with the interdisciplinary team to support positive resident outcomes.
  • Serves as the medicating nurse on an as needed basis; as directed by the Director of Nursing.
  • Participates in the quality assurance and performance improvement activities as requested.
  • Other duties as assigned.
