Clinical Documentation Specialist Outpatient
About The Position
Responsible for reviewing outpatient medical records for documentation impacting coding assignment. This role involves completing reviews of provider-based medical records, conducting follow-up reviews, and prompting/querying physicians regarding missing, unclear, or conflicting medical record documentation, particularly concerning HCC (Hierarchical Condition Category) diagnoses. The specialist will collaborate with coding staff to ensure accuracy of diagnostic data, educate patient care team members on documentation issues, analyze and trend data, and facilitate collaboration between physician, ancillary, and coding staff regarding medical record documentation.
Requirements
- Associates degree
- Current Illinois license for RHIT (Registered Health Information Technician) or CCS (Certified Coding Specialist)
- 2 years of Technical Experience
Responsibilities
