Requirements

  • 2 years of nursing or clinical coding experience
  • High School Diploma or GED
  • AHIMA-RHIA (Registered Health Information Administrator) OR AHIMA-RHIT (Registered Health Information Technician) OR AHIMA-CCS (Certified Coding Specialist) OR AAPC-CIC (Certified Inpatient Coder) OR RN (Registered Nurse)
  • Day-to-day work requires reviewing clinical documentation to identify gaps impacting code assignment and data integrity; applying knowledge of clinical conditions, documentation standards, and coding principles to ensure accurate and complete records; initiating queries for clarification; supporting alignment between documentation and coding; and maintaining productivity and documentation review accuracy.

Responsibilities

  • Reviews clinical documentation to ensure accuracy, completeness, and alignment with coding and regulatory requirements, supporting appropriate reimbursement and data integrity.
  • Reviews medical records to identify documentation gaps impacting coding accuracy and data integrity.
  • Initiates queries to obtain clarification and ensure complete and accurate documentation.
  • Applies clinical, documentation, and coding knowledge to support accurate medical records.
  • Maintains productivity and quality standards while managing assigned work queues and supports alignment between documentation and coding.
  • Works with providers, coders, and care teams to support accurate documentation.
  • Communicates documentation needs and query requests clearly.
  • Uses clinical knowledge to identify documentation gaps and escalate appropriately.
  • Completes documentation reviews and queries timely while maintaining quality.
  • Follows CDI workflows and supports consistent documentation practices.
  • Supports accurate documentation to ensure appropriate coding and reimbursement.
  • Adapts to updates in documentation and regulatory requirements.
