Requirements

  • Bachelor's degree (or equivalent experience) plus minimum of 1 year experience in human services required
  • Strong interpersonal and communication skills
  • Ability to handle sensitive situations with professionalism and compassion
  • Ability to work flexible hours, including evenings and weekends if required
  • $100,000/300,000 Automobile Liability Insurance
  • Valid driver’s license and access to a safe operating vehicle during scheduled working hours
  • Criminal, motor vehicle, and Bureau of Elderly and Adult Services background checks that meet agency standards
  • Completion of Management of Aggressive Behaviors (MOAB) training part 1 and part 2

  • Provide personal care and behavioral support, including but not limited to physical assistance
  • Maintain professional demeanor in emotionally charged situations
  • Ability to write detailed accounts of incidents and behaviors
  • Ability to successfully complete medication training if applicable
  • Demonstrates flexibility in scheduling and daily assignments
  • Demonstrate proficiency in Microsoft Office, including Word, Excel, Outlook, and the ability to master new software

Nice To Haves

  • 3 years experience preferred
  • Required state trainings through Relias Learning or equivalent preferred

Responsibilities

  • Assist clients with daily living activities, including personal hygiene, medication management, and mobility support as needed
  • Implement individualized care plans and follow program schedules under the supervision of their team and program manager
  • Responsible for meeting all Residential Standards and Regulations as required by the Division of Mental Health and Developmental Services and/or Moore Center Services, Inc.
  • Monitor and document clients’ health status, behaviors, and progress in a timely/accurate manner
  • Communicate effectively with clients, clinical supervisory team and all other stakeholders to ensure seamless translation of knowledge
  • Provide emotional support and encouragement to promote independence and well-being
  • Collaborate with team members to ensure comprehensive and consistent care
  • Maintain a safe, clean, and supportive environment for clients
  • Follow all safety protocols and confidentiality policies in accordance with healthcare regulations
  • Adhere to the NH state law requiring mandated reporting of suspected abuse, neglect, or exploitation
  • Other duties as assigned by supervisor
