Covers inpatient or outpatient positions as needed to cover for PTO and FMLA coverage. Assesses the nutrition status of assigned patients and implements all components of the nutrition care process, according to food and nutrition department policies and local regulatory standards (IDPH, TJC, CMS, etc.). Monitors and evaluates nutrition interventions, makes appropriate adjustments to plans as needed. Assesses, interprets, plans, monitors, and documents the nutritional status and nutritional needs of patients using evidence-based standards of care, policies and practice guidelines. Performs a nutrition-focused physical exam on patients. Identifies and provides age-specific nutrition care and meets the cultural needs of the patients and their families. Works within an inter-disciplinary team to provide a plan of care to achieve the patient’s health goals. Educates patients, caregivers, students, and other health care colleagues on the effect of therapeutic diets and nutrition support to maintain/improve nutritional status. When assigned, provides support and supervision to the daily infant formula preparation routine which includes identifying and calculating recipes to individualize the formula to meet nutritional needs of the infant. Participates in department activities to enhance patient satisfaction. Keeps current on nutrition research and incorporates into daily practice. Other duties as assigned.
