Nice To Haves

  • Nutrition Consults are addressed within the day following consult request and consult completion is documented in the electronic medical record (EMR).
  • Nutrition Assessments for San Juan Regional Medical Center (SJRMC) and San Juan Regional Rehabilitation Hospital (SJRRH) patients identified at nutrition risk based on the Nutrition Screen or the Malnutrition Screening Tool are completed within two days and are entered into the EMR.
  • The plan of care for patients identified as at nutrition risk is re-evaluated and documented within at least 1 week of the initial assessment and as necessary based on the plan of care.
  • Document education in the EMR within 24 hours of service.
  • Provide documentation of individual medical nutrition therapy to provider office within one week.
  • Attempt to return calls within 24 hours of receipt.

Responsibilities

  • Provide nutrition education to inpatients and outpatients at SJRMC, SJRRH, and the Cancer Center to facilitate self-management of medical nutrition therapy.
  • Design and present classes (e.g. cardiac, diabetes, pulmonary, weight management) with current nutrition information for patients, providers, and graduate nursing students.
  • Provide education to patients/caregivers at a level that that is understandable to them.
  • Provide documentation of individual medical nutrition therapy to provider office within one week.
  • Develop and present timely nutrition topics based on community need.
  • Coordinate with marketing if appropriate and complete the community report form.
  • Attend committee meetings as an active participant, report progress to clinical team, and serve as an education resource to other staff.
  • Participate in projects involving clinical and information technology overlap; assist with staff training as assigned.
  • Respond in a timely manner to phone inquiries.
  • Enhance professional growth and development to maintain current working knowledge, CDR registration and state licensure.
  • Attend department in-services and meetings.
  • Participate in QA/PI programs when assigned.
  • Complete all annual HealthStream modules 100%.
