About The Position

South Shore Health is seeking a talented and dedicated Clinical Dietitian to join their Clinical Nutrition team. This role provides clinical nutrition services, including medical nutrition therapy, to patients and is an integral part of the healthcare team across all hospital service areas. Nutritional practice is research-based, utilizing current scientific literature, with collaborative goals of care that incorporate Patient & Family Centered care. Strong skills in nutritional assessments using the ADIME method are essential. Responsibilities may extend to nutrition consultations for patients in the SSVNA Home Care Division, including home visits, and potentially covering the Cardiac/Pulmonary Rehab department or the out-patient GDM clinic as needed.

Requirements

  • Bachelor or Master's Degree in Nutrition from a CDR/A.N.D. approved program.
  • Completed Dietetic Internship from a CDR accredited program.
  • Passed RD exam or eligible to take RD exam.
  • Massachusetts licensure or licensure eligible.
  • Proficiency in Microsoft Office Word and Excel preferred.
  • Typing ability.

  • Up to six (6) months to become fully proficient is preferred.

Responsibilities

  • Screens patients for nutritional risk using department standards, EMR data, consultations, and daily multi-disciplinary rounds.
  • Collaborates with Nutrition Guest Representatives, nurses, medical providers, and SLP to identify patients needing assessment or re-assessment.
  • Prioritizes patients for the shift and documents the need for assessment notes and the reasons.
  • Assesses patients using the Nutrition Care Process, utilizing information from EMR, daily meal rounds, patients, families, physicians, nurses, and other clinical providers.
  • Conducts accurate, appropriate, and evidence-based assessments, with timely and effective notes using the ADIME outline and calculations based on N&FS department policies.
  • Documents assessments and recommendations using appropriate EMR templates, ensuring timeliness and accuracy.
  • Effectively communicates departmental, organizational, and industry information to coworkers and other clinical staff in a clear, timely, and productive manner.
  • Provides in-service education to RD staff or other facility employees/students as requested.
  • Functions proactively to resolve patient problems and ensure operational excellence for efficient and effective department services.
  • Works cooperatively with other employees to enhance patient nutritional care.
  • Initiates research of scientific literature to update educational handouts, policies, procedures, standards of practice, or guidelines.
  • Seeks knowledge, skills, and competency to maintain registration with CDR, MA license, and an approved Professional Portfolio.
  • Attends lectures, seminars, and reviews professional journals.
  • Actively participates in department and hospital meetings.
  • Shares responsibility for community educational fairs and presentations.
  • Embraces technology and electronic tools for work processes and efficiency, including computer use for literature searches and Outlook for communication.
  • Provides out-patient counseling for the GDM Clinic, coordinating activities to support hospital goals and potentially becoming a provider for medical insurers.
  • Maintains a neat, organized, clean, and attractive office space for out-patient counseling.
  • Completes administrative duties, documents notes, and sends letters to MDs in a timely manner.
  • Orders educational forms from Printshop in advance of need.
  • Completes and processes patient billing and reconciliation daily.
  • Maximizes patient appointments within scheduled hours and establishes good rapport with scheduling, pre-registration, and UC receptionists.
  • Assists with the out-patient Performance Improvement plan as directed by the CNM, including setting goals, collecting and summarizing data, deciding on action plans, and presenting to the CNM quarterly.
  • Participates in continued learning and possesses a willingness and ability to learn and utilize new technology and procedures.
  • Embraces technological advances for effective and efficient communication.
  • Performs other duties as required by the manager or director.
  • Performs work of varying skill levels and responsibility as directed, potentially including work of a higher or lesser level based on department needs.
