About The Position

Upperline Health is seeking a Clinical Registered Dietitian to provide nutritional services to their high-risk, complex care patient population. This role involves counseling and nutritional assessments as a member of the interdisciplinary team to improve results and maximize outcomes. The ideal candidate will have experience working with an older adult population (65+) and addressing topics such as weight loss, CKD, diabetes, heart health, gout, and kidney stones. Candidates must demonstrate compassion for patient needs, have great attention to detail, and thrive in a collaborative environment. Active FL licensure is required.

Requirements

  • Registered Dietitian in FL required
  • Licensed Dietitian
  • Experience in adult clinical area preferred, or at least 2 years work experience in adult clinical dietetics
  • Experience working with patients with complex, unmanaged Diabetes, Hypertension, and other chronic illness
  • Candidates should be excited and energized by the prospect of working collaboratively as part of an innovative team

Nice To Haves

  • GA licensure a plus
  • Member of the American Dietetic Association preferred
  • Prior healthcare experience serving chronically ill patients living with limited resources and/or mental illness preferred
  • Virtual (phone or video chat) case management or care coordination experience preferred

Responsibilities

  • Educate the interdisciplinary team on dietary and nutrition guidelines to support health maintenance in the older adult population with chronic illnesses.
  • Obtain and evaluate nutrition histories of patients regarding food habits; assess patients' knowledge of diet and health maintenance and provide appropriate detailed education to patients and their families regarding nutritional status, adequacy measures, fluid status, and all nutrition-related lab parameters pertaining to their disease process.
  • Complete written nutritional assessments as well as comprehensive patient assessments for patients' written care plans in accordance with established Federal and State licensure guidelines; participate in care plan meetings.
  • Monitor patients' adherence and response to nutrition therapy, and adjust therapy as needed to achieve and sustain an effective nutritional status; document patients' response to therapy in progress notes.
  • Complete other related duties as deemed necessary by Upperline Health.
