About The Position

Valor Healthcare is seeking a passionate Dietitian to join their team at the Community Based Outpatient Clinic (CBOC) in Cookeville, TN. This role involves providing prescribed medical treatment and personal care services to patients with diseases and injuries, as directed by a physician or mid-level provider. The Dietitian will collaborate with the core PACT Team (Primary Care Provider, RN, and Medical Assistant) and the expanded PACT Team, including family/caregiver, VA, and community-based services, to develop patient care plans. This is a part-time position, requiring two days of work per week.

Requirements

  • Bachelor's degree, or higher, in Nutrition from an accredited school.
  • Must comply with and maintain all requirements for a valid, unrestricted license in the state of desired employment, or in any U.S. state or territory, depending on VA contractual requirements.
  • Must be credentialed and remain in good standing through the Veterans Health Administration (VA).
  • Current certification in Basic Life Support (must be renewed periodically as specified by the certifying agency – AHA valid for two years, e.g.) and in ACLS as specified by individual VAMC contract.
  • Proficiency in written and spoken English.
  • Strong computer skills, including EMR experience.
  • Energetic and optimistic demeanor.
  • This is considered a safety sensitive position.

Nice To Haves

  • Master’s degree preferred.
  • Two (2) years' experience with adult patients preferred.

Responsibilities

  • Educate patients on how to select food to maintain and promote health as well as to prevent illness and disease.
  • Plan appropriate diets and menus to meet the needs of individuals whose medical condition can be treated or controlled using diet therapy.
  • Conduct nutrition education classes.
  • Serve as a community resource to patients.
  • Create in-house nutrition education materials for use in nutrition education of staff and patients.
  • Provide information and education to staff members using nutrition education materials, or in-service/meetings per request.
  • Monitor and respond to phone and e-mail messages promptly.
  • Input all patients’ information into VISTA/CPRS.
  • Complete any and all clinical reminders “due” at the time of each patient visit.
  • Assist in co-managed care collaboration with non-VA providers and facilities.
  • Maintain confidentiality of veterans’ data and information.
  • Comply with all VA and company training requirements.
  • Fulfill compliance requirements of the Office of Inspector General (OIG), Joint Commission (JC), Environment of Care (EOC) oversight, lab compliance and other related items.
  • Remain focused on achieving excellent clinical outcomes through the specified VA guidelines.
  • Provide excellent customer service to each veteran and his/her family, both in person and over the phone, as well as to fellow colleagues and clinic visitors.
  • Participate in the clinic’s outreach events to help support the clinic’s enrollment initiatives.
