The Clinical Dietitian, Homes delivers evidence-based nutrition care to residents in long-term care, rehabilitation, hospice, and dialysis programs. Working as part of a collaborative interdisciplinary team, this role conducts accurate nutrition assessments, develops individualized care plans, and supports ongoing monitoring to promote optimal clinical outcomes. The Clinical Dietitian partners closely with clinical staff and care teams to ensure nutrition interventions align with overall treatment goals and regulatory requirements. Through precise documentation, effective communication, and a team-centered approach, this position supports high-quality care delivery and consistent nutrition practices across home-based and residential settings. Key Responsibilities: Conduct comprehensive nutritional assessments using clinical data, laboratory values, weight trends, and disease-specific guidelines. Develop, implement, and adjust individualized nutrition care plans, including therapeutic diets, supplementation, enteral feeding, and hydration strategies. Monitor resident outcomes and modify interventions in response to medical changes. Collaborate with interdisciplinary care teams to ensure coordinated, compliant, and resident-centered nutrition services. Provide targeted nutrition education to residents, families, and staff to support disease management and diet adherence. Maintain accurate, timely documentation and support audits, surveys, and quality improvement initiatives. Contribute to community outreach and consultative services through education and professional expertise.
