VNA is looking for a Clinical Coordinator. Job Summary: The Clinical Coordinator is responsible for identifying and coordinating nursing care needs for patients and their families, while providing ongoing supportive care to patients, caregivers, and loved ones. In addition, the Clinical Coordinator directs and coordinates care for palliative patients, ensuring appropriate placement within VNA service lines as patient needs evolve. Using the nursing process, the Clinical Coordinator focuses on delivering comfort, comprehensive care, and maximizing quality of life for both patients and families.

Requirements

  • Current Registered Nurse licensed to practice in the State of California
  • Current CPR certification
  • 2 years of Home Health and/or Hospice experience
  • Current driver's license
  • Auto insurance
  • Reliable vehicle

Responsibilities

  • Identifying and coordinating nursing care needs for patients and their families
  • Providing ongoing supportive care to patients, caregivers, and loved ones
  • Directing and coordinating care for palliative patients
  • Ensuring appropriate placement within VNA service lines as patient needs evolve
  • Using the nursing process to deliver comfort, comprehensive care, and maximize quality of life for both patients and families
