About The Position

CFKC is currently looking for a full-time Clinical Coordinator for our Downtown Orlando clinic. Under the direction of the Nurse Manager, the Clinical Coordinator is responsible for planning, implementing, coordinating and participating in the nursing care of in-center hemodialysis patients. Assists in directing the activities of the RN's, LPN's, technicians and other ancillary employees assigned to the hemodialysis unit. This dynamic position requires open availability, a self-starter attitude, excellent interpersonal skills, and a drive to succeed.

Requirements

  • Current Florida R.N. license.
  • Completion of BLS and/or ACLS and/or PALS
  • Associate's degree (A.S.N.) or BSN preferred.
  • Demonstrates ability to function in a leadership position, requiring minimal supervision.
  • Has at least 12 months experience in clinical nursing and an additional 6 months experience in nursing care of the patient with permanent kidney failure or undergoing kidney transplant, including training in and experience with the dialysis process; or, has 18 months of experience in nursing care of the patient with a kidney transplant, including training in the dialysis process.
  • Demonstrates ability to function in a leadership position, requiring minimal supervision, initiative, and a positive outlook.

Responsibilities

  • Responsible for the planning, organizing, implementing and directing of nursing care delivered to the patients in the hemodialysis unit.
  • Responsible for determining patient schedules by coordination of appointment times for staffing efficiency.
  • Addresses and resolves staff schedule issues in a timely manner, participating in direct patient care when necessary to adhere to staff/patient ratio.
  • Coordinates staffing issues with Home Training.
  • Facilitates physician rounding and works closely with the Nurse Manager to ensure open communication with physicians.
  • Must be able to communicate appropriately and responsibly to the physicians for any patient issues, and respond accordingly to critical labs and emergency situations, as well as keeping the Nurse Manager informed.
  • Responsible for managing the patient care staff under the direction of the Nurse Manager and will follow the CFKC process for staff when violations of policies and procedures have occurred.
  • Under the direction of the Nurse Manager, coordinates in-service attendance and equipment evaluation projects in the clinical area.
  • Understands and provides oversight of the clinical staff water room responsibilities.
  • Assists the Nurse Manager with audits for compliance with P&P.
  • Responsible for ensuring that monthly patient care plan meetings are held with the multidisciplinary team and any/all changes are implemented according to policy.
  • Responsible to oversee that all consents for treatment are signed prior to patients' first treatment, working collaboratively with the Social Worker to ensure all paperwork is completed.
  • Ensures the safe keeping and confidentiality of all records and information systems including but not limited to patients, employees and company records.
  • Monitors HIPAA compliance and reports potential problems to the Nurse Manager and HIPAA Officer.
  • In the absence of the Nurse Manager, the Clinical Coordinator will exercise normal prudent management decisions regarding the operations of the facility, calling upon the Director of Clinical services for assistance when needed.
  • Participates in multi-disciplinary patient care conference.
  • Participates in patient and staff education.
  • Assists in the initiation and implementation of the patient and the family education for rehabilitation and/or home health care needs.
  • Plans, schedules, coordinates and explains tests and procedures provided by other medical facilities as necessary.
  • Will work collaboratively with the Nurse Manager in the CQI process of monitoring patient outcomes and reporting to the CQI committee.
  • Upholds and supports company mission in words and actions.

  • Serve as a role model and mentor for staff, as well as support continuing education initiatives.
  • Must be capable of handling a code situation.
  • Other duties as assigned by the Nurse Manager.
