About The Position

Facilitates communication between sending and receiving facilities, including the use of telephone, computer software, and other communication and documentation tools. Documents a record of the encounter. Utilizes clinical expertise, critical thinking, and demonstrated knowledge of unit-specific admission and discharge criteria.

Requirements

  • Current and valid New Jersey Registered Nurse license is required
  • RN graduate of an accredited school of nursing.
  • BLS/CPR certification by American Heart Association is required
  • At least 2 years of clinical experience in an acute care setting required

Nice To Haves

  • Critical care and/or emergency nursing experience preferred
  • Transfer center experience preferred
  • BSN preferred

Responsibilities

  • Answers telephones in accordance with established policies and procedures.
  • Utilizes professional tone and demeanor in all communication.
  • Coordinates communication between sending and receiving facilities, and within the receiving facility.
  • Accurately records details of all communication related to each transfer request in the electronic health record in an efficient and timely manner.
  • Works in a collaborative manner with other members of the health care team.
  • Utilizes clinical expertise, and collaborates with capacity management, nursing, physician/provider representatives, and other hospital departments to assure proper level of care and room assignment at the receiving facility.
  • Maintains awareness of bed availability on all patient care units and outpatient departments involved in the inter-facility transfer and direct admission of patients.
  • Demonstrates the ability to handle a high degree of pressure, heavy workloads, multiple requests, and numerous interruptions in a positive manner, establishing priorities for effective work completion. Able to diffuse challenging situations and respond appropriately.
  • Demonstrates expertise in working with multiple computer applications as well as all functions of the telephone system.
  • Demonstrates the ability to independently implement solutions to transfer center issues.
  • Escalates barriers and concerns using applicable policies and procedures as well as chain of command.
  • Follows patient identification policies and procedures to ensure accurate patient identification. Collaborates with patient access services as needed.
  • Utilizes downtime procedures for effective communication and documentation when necessary.
  • Collaborates with Emergency Medical Services (EMS) to arrange appropriate and timely transportation based on established protocols.
  • Collaborates with sending and receiving providers to ensure appropriate level of care and response urgency are assigned for patient transportation.
  • Obtains insurance information and communicates with insurance providers and/or case management and social services as needed.
  • Initiates registration activities and completes registration as requested.
  • Prepares handoff and other reports as directed.
  • Mentors team members. Participates in onboarding of new team members.
  • Maintains a clean and sanitary work environment.
  • Other duties as assigned.
  • Patient age specification: Neonate, infant, toddler, pre-school, school age, adolescent, adult, geriatrics.
  • Must be able to sit and work at a computer with multiple monitors while speaking on a telephone for extended periods of time.
