The Clinical Coordinator will maintain an active caseload of up to 20-25 individuals referred for START Services. This role involves providing systemic consultation based on assessment and outreach findings, utilizing ecomaps, and offering consultation to systems/teams as needed, adhering to best practices. The coordinator will escalate concerns to the supervisor and/or Clinical or Medical Director as appropriate. Responsibilities include conducting regular outreach visits (in-person and/or telehealth) to individuals' homes, day programs, or vocational settings to complete evaluations and assessments. The role also involves assisting with referrals for consultation and treatment, screening for enrollment in START, completing intake assessments, and referring to the clinical team, Medical or Clinical Director, or Resource Center. The Clinical Coordinator will ensure the coordination of support meetings and crisis plan development using the START Action Plan, providing information and coordinating responses with the Medical and/or Clinical Director. They will develop Comprehensive Service Evaluations, Cross-Systems Crisis Prevention and Intervention Plans, intake/assessments, intervention and outcome plans, and manage admissions/discharges for the Resource Center. Participation in recurring consultations, staff meetings, and meetings with START leadership and the clinical team is expected. The position requires providing on-call support on a scheduled, rotational basis as part of the START team. Accurate and timely completion of all documentation, paperwork, and records, including incident reporting and data entry into the START Information Reporting System, is essential. The role also involves regular outreach with community partners, developing materials, and presenting at Clinical Education Team Meetings. Participation in the Center for START Services National Network, including conference calls and an annual conference, is part of the role. Completing necessary trainings for compliance with YAI policies, NCSS, and state regulatory agency requirements, including obtaining START Coordinator certification, is mandatory. Compliance with all Federal, State, Local, and HIPAA regulations is required. Performs all other duties as assigned.
