About The Position
Provides inmate care coordination and case management. Exercises advanced clinical judgment and accountability for ensuring safe, effective, and compliant healthcare delivery for medically complex and high-risk populations. Coordinates with internal and external stakeholders to optimize patient outcomes, ensure regulatory compliance, and steward Commonwealth healthcare resources. Assignments to other duties or locations may be necessary to meet the clinical and administrative needs of the Department.
Requirements
- Knowledge of clinical coordination and case management of patient continuum of care needs.
- Principles and practices of correctional nursing and health care administration within the Department of Corrections as well as clinical principles and practices.
- Demonstrated ability to communicate effectively orally and in writing throughout the organizational structure.
- Adequate knowledge of computer programs relating to job assignment.
- Ability to interpret and follow oral and written policies/regulations.
- Ability to promote positive public relations.
- Demonstrated ability to organize and complete work assignments.
- Work independently with minimal guidance.
Nice To Haves
- Working knowledge of current ACA standards and compliance documentation.
- Working knowledge of security enhancement to ensure smooth operation between medical departments and other organizational units.
- Case management or similar certification.
