The Clinical Coordinator (RN) will be responsible for the case management of sub-specialty, pediatric patient populations, coordinating their care. This role involves planning, monitoring, and coordinating individual patient care plans, utilizing critical pathways and best practice guidelines for both inpatient and outpatient needs. The coordinator will collaborate with patients, families, specialists, pediatricians, and the multi-disciplinary care team to ensure appropriate, safe, cost-effective, timely, and efficient care. They will also organize, educate, and schedule procedures, appointments, and nurse visits for pediatric sub-specialty patients. Additionally, the Clinical Coordinator will act as the primary point of contact for patients and families, assisting with all aspects of care, triaging concerns, providing medication education and refills, and handling other requests within their scope. This role also includes participation in continuous quality improvement processes, data collection and analysis, and documenting in the patient's medical record. The coordinator will also assist in initiating and implementing necessary changes in patient care delivery by sharing research and current trends.
