The Clinical Coordinator (RN) - Pediatric Specialty Clinic is responsible for the case management of sub-specialty, pediatric patient populations, coordinating their care. The coordinator collaborates with patients, families, specialists, pediatricians, and the multi-disciplinary care team to ensure safe, cost-effective, timely, and efficient care. They also organize, educate, and schedule pediatric sub-specialty procedures, appointments, and nurse visits. Additionally, the role involves coordinating patient care conferences and educational offerings, acting as the primary point of contact for patients and families, and participating in continuous quality improvement processes. The Clinical Coordinator documents in the patient's medical record and assists in implementing changes in patient care delivery through research and current trends.
