About The Position

Since its founding as a teen drop-in center in 1972, Community Solutions (CS) has grown in both depth and breadth of services in response to the growing and changing needs in the community. As one of the most comprehensive nonprofit human services agencies in Santa Clara County and the surrounding areas, CS provides prevention, intervention, treatment, and residential services to more than 6,400 clients annually, helping them overcome the challenges posed by mental health issues, substance abuse, trauma, severe family dysfunction, sexual and domestic violence, and human trafficking. Under the general supervision of the Program Manager, the Clinical Coordinator III – Behavioral Health provides clinical consultation and direction of client care and referrals to the staff, develops and maintains competent, quality client services and well-coordinated community education efforts to ensure contract requirements are met. The Clinical Coordinator III – Behavioral Health collaborates with public and community-based agencies, as needed, to ensure quality service delivery. Services are provided throughout Santa Clara County.

Requirements

  • Master’s Degree in mental health profession and a minimum of two years post licensure.
  • Experience working with mental health and/or drug and alcohol issues.
  • In collaboration with various County services, experience working with systems to meet requirements of the Agency and contracts.
  • Must be able to receive a criminal records clearance as required by specific programs, licensing regulations and Community Solutions practices.
  • Obtain and remain qualified for County 5150 Hold authorization.
  • Requires an automobile to perform routine job duties.
  • Must possess a valid California driver’s license, current care insurance and a good driving record as documented by a DMV report and consistent with the Agency’s liability insurance requirements.
  • Licensed as a Clinical Social Worker (LCSW) through the California Board of Behavioral Science (BBS).
  • Required to maintain current licensure, remain active and in good standing with the BBS, and in compliance with BBS Clinical Supervisory Regulations.
  • Candidates for this position must be legally authorized to work directly as an employee for any employer in the United States without visa sponsorship.

Responsibilities

  • Provides leadership and program coordination to ensure the needs of clients are met and services meet contract and/or certification requirements.
  • Coordinates staff and program’s clinical activities and reports/updates Program Manager and Coordinators on status.
  • Assigns schedules and monitors caseloads in partnership with managers, staff, interns and volunteers.
  • Reviews charts on a weekly basis with staff to ensure audit ready charts.
  • Supports audit practices for the program.
  • Emails staff and copies the Director, managers and admin support on the outcome of auditing results with follow up email for any needed corrections with reports/updates to Program Manager.
  • Provides Program Manager with weekly program status on meeting documentation requirements and timelines.
  • Assists staff with supervision, training and development of all Medi-Cal required documentation; provides weekly clinical supervision Agency wide.
  • Responsible for coordination for clinical care to provide for clients’ stated needs and goals.
  • Provides clinical guidance development of interns, trainees and waivered employees.
  • May provide clinical supervision agency wide.
  • Conducts initial clinical orientation, training and development.
  • Responsible for all required documentation and reporting required by the academic programs, in compliance with Board of Behavioral Science.
  • Enforces all Agency protocols, including Medi-Cal and non Medi-Cal procedures.
  • Provides feedback on response forms to referral sources, follows up on pending referrals, ensures timelines are met and referrals are opened and discharged within program timelines.
  • Coordinates and monitors program Utilization Review (UR) process.
  • Conducts clinical review of all charts including opening and closing.
  • Responds to phone calls and email within 24 hours on business days unless extenuating circumstances exist.
  • Resolve outstanding issues collaboratively using strength-based solutions focused approach.
  • Attends and participates in Agency meetings and contributes to continuous quality improvement activities to enhance team and program development.
  • Consistently uses strength-based and solution focused problem-solving skills.
  • Maintains excellent customer service skills with internal and external customers.
  • Leads regular meetings with program staff and/or volunteers in order to train, schedule services, plan, anticipate and overcome obstacles and share information as assigned.
  • Attends all County/community meetings as assigned and shared pertinent information with the team.
  • Responds to clinical crisis situations and assists staff, clients and their families with problem resolution.
  • Performs involuntary hospitalization assessments and arranges for hospitalization, if necessary, may serve as an expert witness in court cases.
  • Conduct screening, intake and assessments.
  • Completes detailed clinical records and other required documentation in accordance with agency, program and regulatory requirements.
  • Reviews and sign off on clinical documentation within Agency timelines for paraprofessional staff, and other staff, as assigned.
  • Reinforces current policies/procedures and recommends changes/improvements as needed.
  • Provides education, training and presentations to staff, volunteers, clients, families and the community.
  • Develops and maintains collaborative relationships with appropriate community organizations and system providers.
  • Successfully completes all training requirements for this position to ensure agency and contract compliance.
  • Frequent travel required.
  • Employee must have the ability to operate their personal or Agency motor vehicle to participate in client visits, provide daily transportation of clients and/or other assigned agency business.
  • May participate in on-call and after-hours rotation, as needed.
  • Flexible schedule may be required to work some evenings and weekends.
  • Performs first aid as needed based on training and certification.
