Requirements

  • 1 year of office/admin experience strongly preferred.
  • Experience answering phone calls required.
  • Experience providing a high level of customer service required.
  • Experience with Microsoft Office Suite required.
  • Excellent organizational skills and attention to detail.
  • Excellent written and verbal communication skills.
  • Excellent organizational and time management skills.

Responsibilities

  • Manage Hospice Medical Records blast for your team and follow up on all requests in a timely manner including providing feedback to team member when requests are completed.
  • Complete all routine plotting duties and incoming plotting requests as needed and in a timely manner.
  • Manage all DME orders, pick-ups and follow-up requests as needed in a timely manner.
  • Call funeral homes daily on any previous day patient deaths and follow death certificate physician verification process.
  • Order x-rays & labs as requested.
  • Fax documentation for respite stays and moves as requested.
  • Serve as support for business development and field clinicians as needed.
  • Process post care coordination documents and update flags in patient charts.
  • Obtain and upload Home safety eval and emergency plan and upload to chart
  • Processing signed Hospice Non-Covered Items Addendum
  • Answer large amounts of incoming calls from patients, families and referral sources and show professionalism and competency in assisting and/or directing calls to the appropriate department.
  • Phone etiquette: Demonstrate excellent customer service and communication skills, maintaining a high level of professionalism and working to establish a positive rapport with every call.
  • Manage full code plotting/follow up and process advanced directives, obtaining provider signatures, faxing to appropriate care team and uploading to chart.
  • Print aide and nurse visit notes weekly for facility patients.
  • Daily printing of consents, POA, and DNR paperwork following previous day admissions for nurse to put in facility binder.
  • Other duties as assigned
