About The Position
Sana Lake is expanding rapidly and is seeking a Clinical Chart Auditor who is passionate about helping individuals and families recover! The Clinical Chart Auditor is responsible for reviewing clinical and medical records across substance use disorder treatment programs to ensure documentation is complete, accurate, clinically appropriate, and compliant with applicable regulatory, accreditation, payer, and organizational requirements.
Requirements
- Minimum of one year of experience in behavioral health or substance use disorder
- Familiarity with ASAM Criteria, Missouri Department of Mental Health requirements, Joint Commission standards, HIPAA, 42 CFR Part 2, and payer documentation expectations
Nice To Haves
- Associate degree in a relevant field
- Experience with an electronic health record system
Responsibilities
- Conduct routine and focused audits of client records across applicable levels of care, including detoxification, residential, PHP, IOP, outpatient, and recovery-support services
- Review documentation completed by counselors, therapists, case managers, nurses, physicians, nurse practitioners, physician assistants, and other members of the treatment team
- Evaluate records for completeness, accuracy, consistency, timeliness, appropriate authentication, and compliance with organizational policies
- Confirm that documentation accurately reflects the services provided, the client’s clinical condition, progress toward treatment goals, and ongoing need for the current level of care.
- Review admission assessments, biopsychosocial assessments, medical evaluations, treatment plans, progress notes, group notes, individual and family therapy notes, case-management documentation, medication records, continued-stay reviews, transition plans, and discharge summaries
