The Clinical Case Manager provides trauma-informed, clinically-oriented case management services to adults with serious mental health conditions who are transitioning from incarceration, probation, or other justice system involvement back into the community. This role integrates clinical assessment, care coordination, and reentry stabilization services, supporting clients in navigating the complex challenges of reintegration, including housing instability, behavioral health needs, substance use recovery, and access to medical and social services. The Clinical Case Manager applies clinical judgment, risk assessment, and crisis intervention skills to promote safety, reduce recidivism, and support long-term recovery. Services are delivered in the field—including homes, correctional reentry settings, and community agencies—as well as via telehealth. Working as part of a multidisciplinary team, the Clinical Case Manager collaborates with probation/parole, behavioral health providers, housing partners, and community-based organizations to ensure coordinated, client-centered care. This role will report directly to the Program Supervisor. This role will uphold and champion the agency’s mission, vision, and values and contribute to a collaborative and inclusive work culture.
