About The Position

Full Circle Health is looking for a Clinical Case Manager (LPN or CMA/RMA) to support our Transitions of Care Program across our clinics! The purpose of the Clinical Case Manager is to work collaboratively with the health care provider(s), interdisciplinary team, patients and community health partners to coordinate care for individuals with chronic or complex conditions, or those requiring high levels of care coordination. The Clinical Case Manager is responsible for assisting with Transitions of Care post hospitalization.

Requirements

• LPN from accredited College of Nursing or completion of an accredited Medical Assistant training program.
• Minimum of two years of clinical practice.
• Currently certified by American Medical Technologists, American Association of Medical Assistants, American Registry of Medical Assistants, Licensed Practical Nurse or other accredited certifying organization.
• Immunizations for all staff to include an annual influenza vaccine.

Nice To Haves

• Previous population health, case management or transitions of care experience.
