About The Position

This is a time-limited position at the University of Florida College of Medicine – Jacksonville. The Clinical Case Manager participates in and conducts developmental evaluations/screenings, consultations, direct services, and family assessments in collaboration with a multidisciplinary team. This role is crucial for completing intake, determining eligibility, monitoring progress, updating Individualized Family Support Plans (IFSPs), and planning/providing intervention. The position requires working with the IFSP team to ensure State and Federal timelines are met regarding service delivery, transition, IFSP development/updates, and LEA/SEA notification. The Clinical Case Manager will also recommend, coordinate, and make referrals to community services to meet identified outcomes on IFSPs, and maintain current and accurate client records, including program plans, intervention notes, case notes, and billing information. Participation in local and statewide meetings and trainings is expected to ensure best practice and compliance, along with completing required trainings/modules.

Requirements

  • Master's degree in an appropriate area; or a bachelor's degree in an appropriate area and two years of relevant experience.
  • Valid driver's license.
  • Level II Background Check is Required.
  • Health Assessment Required: Yes

Nice To Haves

  • Bachelor’s degree in Early Childhood or Early Childhood Special Education, Social Work, Psychology, or Sociology.
  • Two years of appropriate experience with children birth to five with special needs.

Responsibilities
