Position Summary: The Clinical Case Manager conducts comprehensive in‑person assessments and develops individualized care plans for members in Medicare/Medicaid Waiver Programs. This role coordinates physical, behavioral, and social services across providers and community resources, ensuring members receive appropriate, high‑quality, and cost‑effective care. The Case Manager authorizes services, documents care activities, participates in interdisciplinary meetings, and supports team development. With flexible teleworking and meaningful community engagement, the position offers the opportunity to build trusted relationships, advocate for vulnerable populations, and apply strong clinical judgment while pursuing ongoing professional growth.
Requirements
- Active Behavioral Health license in IL – LCSW, LCPC, LMFT
- Must reside in the state of Illinois near Zion, Waukegan, or the Lake County area
- Minimum of two (2) years of case management experience
- Microsoft Office and electronic health record experience
- Must possess reliable transportation and be willing and able to travel up to 75% of the time.
