Behavioral Health Clinical Case Manager – Field in Ocean and Atlantic Counties, NJ (Telework + Field-Based Travel) Join our LTSS team as a Behavioral Health Clinical Case Manager, where you’ll play a vital role in improving the lives of our members by connecting them with the care, resources, and support they need to thrive. This is a unique opportunity to blend clinical expertise with care coordination in a flexible, telework environment—while still getting out into the community to make a real impact.
Requirements
- Licensed Clinical Social Worker (LCSW) or Licensed Social Worker (LSW)
- Minimum 2+ years of behavioral health experience
- Must possess reliable transportation and be willing and able to travel up to 75% of the time. Mileage is reimbursed per our company expense reimbursement policy
- Ability to successfully complete NJ Choice Certification (required for continued employment)

- Experience in case management or discharge planning
- Managed care experience
- Strong crisis intervention skills
Responsibilities
- Conduct comprehensive, face-to-face assessments for members enrolled in MLTSS and D-SNP programs
- Develop and implement personalized care plans that address physical, behavioral, and psychosocial needs
- Collaborate closely with members, families, providers, and interdisciplinary teams to coordinate care
- Evaluate and facilitate services such as: Adult and pediatric medical daycare, Personal care assistance, Nursing facility support, Community-based and long-term services
- Advocate for members to ensure access to appropriate and high-quality care
- Authorize services and ensure efficient, cost-effective resource utilization
- Document care activities accurately in the electronic health record
- Participate in interdisciplinary meetings and help drive positive outcomes
- Support and mentor new team members as you grow in your role
