About The Position

The Case Manager plays a vital role in delivering client-centered services that support patients across the continuum of care. This position partners with patients and their families to coordinate healthcare and connect them with appropriate home- and community-based services. You will ensure timely access to medically appropriate care while promoting continuity, quality outcomes, and a positive patient experience. We are seeking a skilled and patient-focused Case Manager (RN) to join our growing team.

Requirements

Active, unrestricted Registered Nurse (RN) license in the State of Florida (must be maintained)
3+ years of clinical experience

Nice To Haves

1+ year of case management experience
Strong computer proficiency, including Excel
Experience with Electronic Medical Records (EMR) systems (Athena experience a plus)
Excellent communication, organization, and problem-solving skills

Responsibilities

Conduct comprehensive patient assessments and develop individualized care plans
Coordinate care across providers and service settings
Advocate for patients and their families to ensure access to necessary resources
Provide education to patients, caregivers, and support systems
Monitor patient progress and adjust care plans as needed
Maintain accurate, timely, and compliant documentation
Deliver age-appropriate care based on developmental needs
