NFI North is seeking a Clinical Case Manager-Transitional Family Specialist for Oliver Place, an adolescent residential program in Bath, Maine. Responsibilities include:  provide family-centered planning and facilitation, develop plans of care driven by the strengths and needs of the child, youth and family/primary caregiver, obtain targeted support services, and monitor the provision and outcomes of the plan, so that the child or youth and family can be successful at home, at school and in the community. Other duties may include: attend treatment meetings, assist clinician with individual and family assessments, treatment plans, court reports and monthly reports, connect participants to community resources and enhance permanent connections for participant.    Transitional Family Specialist will participate in weekly management and staff meetings, provide on call support, and work as part of a multi-disciplinary treatment team.

Requirements

  • Applicants must have a BA in Social Work, Psychology or related field and experience working with individuals with mental illness and their families.
