Requirements

  • Associate degree in nursing from an accredited college or university
  • One (1) year of nursing experience
  • Completion of a PHN course within one year of hire
  • Current Georgia Registered Professional Nurse license
  • CPR certification

Responsibilities

  • Provides comprehensive case management services.
  • Responsible for coordinating care, monitoring treatment adherence, facilitating access to healthcare services, and ensuring timely follow-up for patients diagnosed with or at risk for tuberculosis, sexually transmitted diseases, and women requiring breast and cervical cancer screening follow-up.
  • Provide comprehensive nursing case management for patients diagnosed with or suspected of tuberculosis (TB), including assessment, treatment monitoring, medication adherence, patient education, and successful completion of therapy.
  • Coordinate and monitor follow-up care for patients with sexually transmitted diseases (STDs) and abnormal mammogram or Pap test results, including patient notification, referrals, treatment coordination, and follow-up to ensure timely completion of recommended care.
  • Provide direct patient care in accordance with Georgia Department of Public Health nurse protocols, utilizing sound clinical judgment to assess, treat, educate, and monitor patients throughout the continuum of care.
  • Perform comprehensive patient assessments, develop and implement individualized care plans, monitor patient progress, coordinate referrals, and collaborate with physicians and other healthcare providers to ensure continuity of care.
  • Educate patients and families on disease processes, treatment plans, medications, preventive health practices, available community resources, and healthy lifestyle choices to support informed decision-making.
  • Maintain accurate, complete, and timely documentation of patient assessments, treatments, referrals, education, consultations, and follow-up activities in the electronic medical record and required public health reporting systems.
  • Ensure compliance with all applicable state and federal regulations, public health reporting requirements, communicable disease protocols, and women's health program standards.
  • Collaborate with multidisciplinary teams, community organizations, and external partners to coordinate patient care, improve access to services, and support district public health initiatives.
  • Participate in community outreach, health promotion, and public education activities to increase awareness of preventive health services and available community resources.
  • Participate in quality assurance and quality improvement (QA/QI) activities, including chart reviews, peer reviews, protocol development, strategic planning, and implementation of new programs and services.
  • Participate in emergency preparedness planning, training, and response activities, and complete all required certifications and annual training, including nurse protocol, HIPAA, Bloodborne Pathogens, and other mandatory education.
  • Perform other duties as assigned, including inventory management, report preparation, clinic support activities, and coordination of community outreach initiatives.
