About The Position

Under the direction of the Service Coordinator Supervisor, the Clinical Case Manager is responsible for conducting screenings, completing family assessments, conducting case management activities, serving as a member of a multidisciplinary team for the purpose of determining eligibility during evaluations, updating IFSPs, coordinating services, making recommendations, and accessing community resources in natural environment settings for children referred to the Northeastern Early Steps Program through Part C of the Individuals with Disabilities Education Act (IDEA). This role also involves maintaining compliance with Local, State, and Federal guidelines for best practice and billing under AHCA and the Early Steps State Office. The position participates in/conducts developmental evaluations/screenings, consultations, direct services, and family assessments in collaboration with the multidisciplinary team when appropriate for completing intake, determining eligibility, monitoring progress, updating IFSPs, and planning/providing intervention. The Clinical Case Manager works with the IFSP team to ensure State and Federal timelines are met with regard to timely service delivery, transition, IFSP development/updates, and LEA/SEA notification. They recommend, coordinate, and make referrals to community services to meet identified outcomes on Individualized Family Support Plans. Additionally, the role involves maintaining current and accurate client records, including program plans, intervention notes, case notes, and billing information, timely and accurately for payment. The position also participates in local and statewide meetings and trainings to ensure best practice and compliance, and completes trainings/modules as required by the program and organization.

Requirements

  • Master’s degree in an appropriate area; or a bachelor’s degree in an appropriate area and two years of relevant experience.
  • Level 2 Background Check is Required.

  • Degrees earned from an education institution outside of the United States are required to be evaluated by a professional credentialing service provider approved by the National Association of Credential Evaluation Services (NACES).
  • Hiring is contingent upon eligibility to work in the US.

Nice To Haves

  • Bachelor’s degree in Early Childhood or Early Childhood Special Education, Social Work, Psychology, or Sociology.
  • Two years of appropriate experience with children birth to five with special needs.
