Requirements

Master’s degree in social work, Counseling, Psychology, or a related field
Active, unrestricted clinical license (LCSW, LSW, LISW, LCPC, LPC, LP, LMFT, LMHC, LMSW, or equivalent)
3+ years of post-master’s direct clinical experience
3+ years of experience treating mental health and substance use disorders
3+ years of case management experience
Experience with crisis intervention and motivational interviewing
Strong technical skills and ability to thrive in a fast-paced, telephonic environment

Nice To Haves

Managed care, care management, or utilization review experience
Discharge planning and care coordination experience

Responsibilities

Conduct comprehensive behavioral health and psychosocial assessments
Develop and manage individualized care plans
Coordinate care with providers and community resources
Support members through crisis intervention, referrals, and ongoing follow-up
Use motivational interviewing and coaching techniques to drive positive outcomes
Identify and address quality-of-care concerns
Maintain accurate, compliant clinical documentation
