About The Position

Halcyon Home is seeking a Clinical Care Transitions Coordinator in Austin, TX. This is a field-based role for a clinically licensed professional focused on building physician relationships and expanding referral networks. The primary responsibility is to promote and sell the organization's Home Health Specialty Programs to physicians and acute care settings. The role involves marketing specialty programs directly to healthcare providers, using clinical knowledge and relationship-building skills to grow partnerships and increase patient referrals.

Requirements

Active RN, LVN, PT, or PTA license required.
Excellent communication and presentation skills, with the confidence to engage physicians and healthcare professionals using appropriate clinical terminology.
Flexible, hands-on, and willing to step in wherever needed.
A caring professional who leads with empathy, integrity, and follow-through.
Valid driver's license, reliable transportation, and willingness to travel within the assigned territory.

Nice To Haves

Experience in homecare, home health, hospice, and/or assisted living strongly preferred.
Previous physician liaison, clinical marketing, healthcare sales, or business development experience preferred.

Responsibilities

Drive referral growth by promoting and selling Home Health Specialty Programs to physicians, hospitals, and acute care providers while building trusted, long-term referral relationships.
Educate physicians and referral sources on clinical services and patient care solutions.
Serve as a trusted clinical resource, confidently communicating with physicians and healthcare professionals using appropriate clinical terminology.
Conduct in-person visits to physician offices and healthcare facilities to develop and maintain strong referral partnerships.
Collaborate with clinical and operations teams to ensure a seamless transition of referred patients into services.
Stay informed on specialty programs, clinical offerings, and industry trends to effectively represent the organization.
Represent the organization professionally at networking events, provider meetings, and other business development opportunities.
